Requirements for extension of visit to the U.S.

  • Legible photocopy of passport
  • Legible photocopy of Visa
  • Legible photocopy of the I-94
  • Address of residency outside the U.S.

    Note: You must apply before your I-94 expires. It is preferable to process this form a month before the expiration date.

Requirements for residency
Petitioner and Beneficiary

  • Birth Certificate
  • Certificate of Marriage
  • Divorce Certificate or Death Certificate (for a marriage petition)

    Note: All documents must be in English and notarized.

Petitioner

  • Certificate of Naturalization or residency card (Green Card)
  • Past three Income Tax reports
  • Letter of employment indicating monthly salary or hourly wages (original)
  • Three most recent bank statements
  • Two photos of residency style

Beneficiary

  • I-94
  • Passport
  • Visa
  • Medical examination
  • Four photos of residency style
  • Police Record (Clearance Letter)

    All documents must be photocopied legibly.

Requirements for Citizenship form N-400

  • Residency card and a photocopy of both sides
  • Social Security card and a photocopy of both sides
  • List of all entries to and departures from the country since obtaining residency with the number of days spent outside the country and the number of trips out of the country in the last 5 years
  • Children: date of birth, alien number, and current address
  • Addresses where you have lived in the past five years
  • Height and weight
  • Spouse: date of birth, social security number, alien number, date of marriage
    If your spouse is a citizen, the date and city where he/she was naturalized

  • If you have been married or your spouse has been married previously or if you or your spouse have been a widow(er) you must also have:
    • Name of the previous spouse
    • Date of marriage
    • Date of divorce or date of death of previous spouse
    • How the relationship ended
    • Immigration status of the previous spouse
  • If you have sent the application for military service it is necessary to have the number corresponding to that application and the date. This applies for males who obtained their residency before age 26.
  • Two citizenship style photos


Exceptions for presentation of citizenship exam in English

  1. Over age 50, resident for at least 20 years – exempt from the exam; may have the interview in the language of their choice.
  2. Over age 55, resident for at least 15 years – exempt from taking the exam in English; may choose what language the interview is conducted in.
  3. Person disabled or with a mental problem – should have medical certificate form N-648 filled out by the family doctor. The disability may NOT be due to drugs.
  4. Fingerprints – not required for people over age 79.
